Eligibility requirements
- Residency: Must be an Indiana resident.
- Age: Must be at least 18 years old.
- Income: Annual household income cannot exceed 200% of the federal poverty level.
- Criminal Justice Involvement: Must have an open criminal case that includes a felony charge or a prior felony conviction.
Important Information
Please read the information below prior to beginning the referral process:
- Participants and Criminal Justice Partners (CJPs) are the only designated entities permitted to submit self-referrals. Agencies and staff are not authorized to complete referrals on behalf of a participant.
- A maximum of one (1) referral may be submitted every 90 days.
- If you are currently incarcerated, you must provide the contact information of a designated representative from your facility to complete this application. The Recovery Works team will coordinate with this representative regarding your referral submission.
- Recovery Works utilizes a variety of factors to match the participant with a provider, including participant input, provider availability, location etc. Recovery Works liaisons are unable to modify or reclassify referrals once they have been distributed.
- All referrals for Recovery Works must be received by the provider prior to the participant beginning services.
