Mission of Vital Records
The mission of the Henry County Health Department’s Vital Records Office is to accurately, securely, and efficiently maintain and issue birth and death records for the public. We are committed to providing timely, respectful, and confidential service while ensuring compliance with state and local laws that protect the integrity of vital records.

To obtain a birth or death certificate, you will need:
- Completed request form (Birth Certificate Request Form or Death Certificate Request Form)
- Valid photo ID
- Payment: $20 per copy (cash, check, credit/debit)
- A 3% convenience fee applies to credit/debit transactions
Please note: Certificates will not be issued without all required items.
