During an annual physical, your Primary Care Provider (PCP) will collect measurements such as blood pressure, cholesterol, and blood sugar levels. These measurements provide valuable information about your current health and can help identify potential health risks.

The health plan pays for one physical every calendar year. Annual physicals at an in-network provider are covered 100%* by the State employee health plan for eligible employees and spouses, AND you can also earn $200 in Wellness Rewards for completing the appointment and submitting the required form.

Find a PCP in your plan:

1. Call your Family Advocate at 1-877-814-9709 and they will search for you.
2. Log in at anthem.com and choose Find Care.
3. Download the Sydney Health app, log in, and select Find Care.

How to earn a $200 annual physical wellness reward

1. Make an appointment with your PCP for a physical with lab work.
2. Print the Annual Physical Results form and fill in Section 1.
3. Bring the form to your appointment and have your PCP complete Section 2.
• If you already completed your 2026 annual physical but didn’t get your form filled out, it’s not too late. Email the form to your PCP to complete and coordinate with them to receive your completed form.
4. You are responsible for submitting your completed form. Follow instructions on the form to submit by uploading to the Mobile Health Consumer portal or by fax. It can take up to 4 weeks for the forms to be processed. Your form is considered processed when the Mobile Health Consumer portal Rewards Center shows the activity as complete.

Don't delay! Results must be visible in the Mobile Health Consumer portal by December 31, 2026, to earn the gift card.
