The Wellness Rewards Program is open to State of Indiana employees and spouses enrolled in a State Personnel Department sponsored medical plan. Employees and spouses who participate in the Wellness Rewards Program can earn wellness rewards.
Why do I have to be a member of the health plan to participate? The Wellness Rewards Program is a coordinated benefit within the health plan; therefore, it is only available to employees, spouses, and adult dependents who are enrolled in a health plan sponsored by the State Personnel Department.
Who can earn wellness rewards? Employees and spouses enrolled in a State Personnel Department sponsored medical plan can earn wellness rewards.
I am a new employee, can I participate? Yes, if you enroll in a health plan sponsored by the State Personnel Department. New employees are encouraged to begin participating in all wellness programs available to them. New employees typically have access to create an account in the Mobile Health Consumer portal 1-3 weeks after health plan coverage begins.How does my spouse earn rewards? Employees and spouses who are enrolled in a State Personnel Department sponsored medical plan can each earn $500 in Wellness Rewards in 2026. Employees and spouses each register their own account on the Mobile Health Consumer portal. Is there a deadline to earn and redeem gift cards? Yes. All gift cards must be earned and redeemed by December 31, 2026. Keep in mind many activities have processing time that impacts how quickly the reward is shown as earned in the Mobile Health Consumer portal. I completed an activity, but the reward does not show earned yet. What is going on? You may be checking too soon. Review the reward details in the Mobile Health Consumer Rewards Center for estimated processing times specific to each reward. Processing time allows for review and verification to ensure reward requirements are completed. If it is outside the listed processing time, contact your Anthem Family Advocate at 877-814-9709.
Will the gift cards be taxed? Yes, gift cards are taxable income based on IRS regulations. The value of the gift card will be included in your gross wages with a description of “Wellness Reward” on a paycheck following the redemption of the gift card.
What is the difference between earning and redeeming a gift card? An earned gift card means that you have completed an eligible activity and you are eligible to receive a gift card. A redeemed gift card means you have logged in to the Mobile Health Consumer portal and used the Store in the Rewards Center to select a retailer to receive your gift card.
How do I redeem my gift cards? Gift cards can be redeemed in the Mobile Health Consumer portal in the Rewards Center. Click on “Store” located under Browse. Select your preferred retailer and the amount you’d like to redeem. Redeemed gift cards can be found in the “Redeemed” section of the Rewards Center.Do I have to earn a minimum amount before I can redeem a gift card? Gift cards can be redeemed in variable increments from $5-$500. Check the Mobile Health Consumer Rewards Center for details on each card.
Is there a limit to the number of gift cards I can redeem? No, there is no limit to the number of gift cards that can be redeemed in 2026.
Who do I contact if I lost or did not receive my gift card? Contact Mobile Health Consumer Support at support@mobilehealthconsumer.com for lost or missing gift cards.Can I get a physical gift card instead of a digital card? No, only digital cards are available. You can select a retailer and then use your digital reward to purchase physical gift cards on their platforms. For example, if you select Amazon for your digital card, you can purchase a physical gift card on Amazon.com.

What is the wellness reward for completing an annual physical? Employees and spouses eligible for the State employee health plan can earn $200 for completing an annual physical with lab work and submitting the Annual Physical Results Form to Mobile Health Consumer.
How long does it take for the activity to show as completed in the Mobile Health Consumer Rewards Center? This activity will show as completed within four weeks of your Annual Physical Results Form being submitted to Mobile Health Consumer and accepted for processing. To be accepted for processing, forms must be legible and fully completed including all required values and required signatures. Annual physical forms should be submitted by November 30 to ensure there is time for processing.What if my health care provider doesn’t recommend lab tests for some of the values on the Annual Physical Results Form? If your provider determines some lab test values are not necessary for you, the provider can note that on the Annual Physical Results Form and the form will be accepted if noted properly Height, Weight, and Blood Pressure are always required to receive the reward.
What is an annual physical? A yearly checkup with your primary care provider to assess your overall health, catch potential issues early, update vaccinations, and build a health history.
What should I expect at an annual physical? At your annual physical, your provider will review your family and medical history, check your vital signs (blood pressure, heart rate), order lab testing, and discuss lifestyle habits. Your provider will answer your health-related questions and offer guidance to improve your overall health and wellbeing.
How do I prepare for an annual physical? Create a list of all medications you currently take, including over-the-counter drugs and supplements. Include a list of any symptoms or pain you are experiencing and any questions you would like answered.

Your annual physical allows you to spend quality time with a provider to ask health-related questions, catch minor issues before they become problematic (e.g., cancers, heart disease, infections), and offers guidance to improve your overall health and well-being.
What is the deadline for earning the annual physical reward? The activity must show as completed in the Mobile Health Consumer Rewards Center by Dec. 31, 2026 to earn the reward. All gift cards must be redeemed by Dec. 31, 2026. Keep in mind processing time is up to four weeks from the date the form is accepted to when the activity is shown as completed in the Mobile Health Consumer portal. Can my employer see this information? No. The annual physical is private and confidential. Your privacy is important to the State Personnel Department and to Mobile Health Consumer.
How much does the annual physical cost? Preventive care is not subject to deductible or co-insurance. An annual physical is considered preventive care so there is no charge to you. Keep in mind providers are required to bill for all services provided during an appointment. If non-preventive care is provided at the same time as the annual physical, there could be out of pocket costs.
How do I get the reward for completing an annual physical? When you call to schedule your appointment, make sure to mention the lab values on the Annual Physical Results Form. Print the Annual Physical Results Form and take it with you to your appointment. You fill out Section 1. Make sure you enter your name as it appears on your insurance card. Your provider fills out and signs Section 2.
Submit the completed form to Mobile Health Consumer by fax to 833-421-6742 or secure upload in the Mobile Health Consumer portal. You will receive confirmation when the form has been accepted. The activity will show as completed in the Mobile Health Consumer Rewards Center once your form is processed. Form processing takes up to four weeks from the date your form is accepted. If you fax the form, keep your fax confirmation until the activity is shown as completed in the Mobile Health Consumer portal.

What is the wellness reward for completing a cancer screening? Employees and spouses eligible for the Mobile Health Consumer program can earn $50 for each cancer screening completed in 2026. Rewardable cancer screenings are breast cancer, cervical cancer, colon cancer, prostate cancer, and lung cancer.
How long does it take for the Cancer Screening activity to show as completed in the Mobile Health Consumer Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem. The activity will show as completed within two to three weeks of your claim being processed by Anthem.

How long does it take for the Vaccines activity to show as completed in the Mobile Health Consumer Rewards Center? This activity is completed based on your provider submitting a claim for the service to Anthem or CVS/Caremark. The activity will show as earned within two to three weeks of your claim being processed by Anthem or CVS/Caremark.
What vaccines are rewardable? COVID-19, flu, hepatitis A and B, HPV, measles, mumps and rubella (MMR), meningitis, pneumonia, chickenpox, shingles, diphtheria, tetanus, pertussis (whooping cough), and respiratory syncytial virus (RSV) are all rewardable vaccines in 2026.
Who should get a vaccine? Check with your provider to see which vaccines are right for you. Why are vaccines important? Receiving vaccines is the best way to prevent illness. Vaccines train your immune system to fight off dangerous viruses and bacteria, preventing serious illnesses, hospitalization, and death by creating immunity before you’re exposed.

Why isn’t my reward for my vaccine showing in the Mobile Health Consumer portal? You may be checking too soon. Vaccine rewards are automatically completed within two weeks of your claim being processed by Anthem or CVS/Caremark. Once you can see your claim has been processed at Anthem.com or Caremark.com, expect to see your reward in Mobile Health Consumer within two weeks.
Although uncommon, sometimes providers do not submit claims right away, or there is a problem with the way a claim was filed. If you’ve waited more than four weeks from the date of vaccine and Anthem or CVS/Caremark still hasn’t processed the claim, you can use the Missing Preventive Care Service process to submit documentation. Documentation will not be processed unless it has been four weeks since the date of your service. This form must be submitted by November 30 to allow for processing time.

Individuals who have a body mass index (BMI) of 25 or higher, do not have type 1 or type 2 diabetes, are not pregnant, and have a diabetes risk factor such as a prediabetic range blood sugar result, previous diagnosis of gestational diabetes, or a family history of diabetes. You will take a Prediabetes Risk Survey prior to starting the program to ensure it is a good fit for you.
What Diabetes Prevention Program options are available to me? Anthem coordinates two programs that are free to health plan members:- Lark app based DPP

- Cardinal: Agencies with 100 to 499 employees
- Firefly: Agencies with less than 100 employees

How do I earn a reward for participating in the Health Assessment Battle Activity?
