Permit Forms and Resources
The INDOT Permits Division has created and assembled forms, guides, resources, and other information useful in the permit application process. These resources, unless otherwise stated, are to be regarded as guidance only and do not constitute legal or technical advice. Bond Forms
Bonds are required for most INDOT permits and must be processed by a qualified surety company. The bond must be of a sufficient amount to cover all proposed work within the right-of-way. Bond forms must be notarized and submitted as a PDF attachment in the Electronic Permit System (EPS) and submitted in hard copy, with original signatures and notarization. Bond forms that are not fully completed, with all signatures, signers identified, and properly notarized with seal, will not be accepted.
Blanket bonds, which are used to cover multiple permits, should be submitted to:

Blanket bonds may be released by emailing the Bond Release Information Request Form to permits@indot.in.gov.
Performance bonds are used to cover a single permit and must have the original documents submitted to the respective district office where work will be taking place. The Bond Release Information Request Form should also be submitted to the respective district office.

Outdoor Advertising Affidavit Templates
The following affidavits may be necessary for the processing of an application for an Outdoor Sign permit. Which affidavits are needed depends on the nature of the specific application in question. Any questions concerning these affidavits and their use can be directed to the respective District Outdoor Advertising Specialist or Permit Investigator.

Boring and Jacking Specification Worksheets
The following worksheets are used when submitting a permit application to INDOT for the purpose of boring and jacking a pipe under a state-controlled roadway or interstate highway.
Additional Forms & Resources
- Used when diverting traffic off a state roadway to a city or county street/road
Agreement to Execute an Access Control Document
- In consideration for INDOT granting a driveway permit, the applicant may be required to sign an agreement to execute an access control document conveying the access rights for the balance of property frontage owned by the applicant. The agreement will allow INDOT to issue a permit to begin driveway construction before the actual deed relinquishing the remaining access rights is prepared and recorded on the property. This document is required for most developments with more than 400 feet of frontage along a state-controlled highway.
Additional Disclosure Form (State Form 23237, R/3-00)
- Used when there are additional parties involved with a proposed project that must be acknowledged. One copy of the Additional Disclosure form is to be notarized and submitted to INDOT with original signatures.
- Used when a design consultant submits a permit application via EPS on behalf of a property owner. This form gives the consultant the right to act on behalf of the property owner.
