Vital Records
Office Hours: 8:00-4:00 PM, closed for lunch 12:00-1:00 PM
Certificate requests can be submitted via US mail, e-mail, or fax. Feel free to contact our office by phone if you have questions at (765) 521-7059, Option 1.
To obtain vital records such as a birth/death certificate we need the following items:
- Request Form
- Photo ID
- $20 per copy requested cash, check, credit/debit accepted (a convenience fee of 3% for credit/debit).
**Please note that the certificate WILL NOT be issued without the above-required items**
