State of Indiana Early Retiree and COBRA Participants
If you decide to elect to continue your coverage, please be aware you are also eligible to make Open Enrollment changes for the 2026 plan year. Similar to an active participant, you may elect to change plans, coverage level, add or remove dependents (if applicable) and elect to participate in the Non-Tobacco Use Agreement (NTUA) for the 2026 plan year effective January 1, 2026.
As a COBRA or Early Retiree participant, your Open Enrollment elections are not completed through PeopleSoft. All changes must be completed by paper including the 2026 Non-Tobacco Use Agreement (NTUA). If you do not wish to participate in the Non-Tobacco Use Agreement or make changes to your health plan election, no action is needed.
IMPORTANT DATES
All changes and additions must be mailed to HRPro postmarked no later than November 19, 2025 and will be effective January 1, 2026.
- Non-Tobacco Use Agreement (NTUA): If you only wish to participate in the NTUA for 2026, you must complete and return the Non-Tobacco Use Agreement to HRPro by November 19, 2025.
- Participants who fail to complete and return the NTUA by the deadline will not be eligible for the premium reduction in 2026.
- Please note: If you do not participate in an Anthem medical plan, you DO NOT need to complete the Non-Tobacco Use Agreement.

- 2026 Open Enrollment changes: If you wish to make changes to your health plan elections for 2026, you must complete and return the State of Indiana Health Care Plan Enrollment Form to HRPro by November 19, 2025.
- Please note: If you elect COBRA or Early Retiree Insurance but do not complete the State of Indiana Health Care Plan Enrollment Form, your current coverage will roll-over.

Early Retirees
Beginning in January 2026, retirees covering a spouse on their medical plan may be required to pay a spousal surcharge. If your spouse has access to medical coverage through their employer and does not elect it for 2026 and you enroll them as a dependent under the State’s medical plan, you will be charged the spousal surcharge. If you do not fall in the above scenario but are covering a spouse on your medical plan in 2026, you will want to provide verification to have the spousal surcharge waived.
This change will apply only in the situation that:
- You cover your spouse under a State Employee Health Plan, and
- Your spouse’s employer offers them health insurance.
- Your spouse is not enrolled in their employer’s health plan.
It will not apply if your spouse:
- Is not employed.
- Does not have access to coverage through their employer.
- Also works for the State of Indiana.
During Open Enrollment, you will need to complete a spousal surcharge form, that may require documentation from your spouse’s employer. The surcharge will apply if you cover a spouse but do not complete these steps. Health care costs continue to rise, and the State must ensure that the State Employee Health Plan (SEHP) remains viable long-term for employees and their dependents.
