1095-B forms
1095-B forms confirm health insurance coverage for employees and their dependents. Due to recent changes under the Paperwork Burden Reduction Act, these forms are no longer required to be automatically mailed or distributed. However, the documents are available upon request.
Due to the carrier switch in October, there are two separate forms for 2025. One is from Anthem for coverage from 1/1/2025-9/30/2025 and the other from ChamberCare from 10/1/2025-12/31/2025. Please utilize the information provided below to obtain the forms:
