2025-2027 Local Community Partnership Grants

Local Community Partnership grant opportunities for the 2025-2027 funding cycle are live.
Applications will be due in March – *please note the timeline in the application document as due dates may vary based on grant*.

There will be two trainings held each day: capacity-building for communities that do not currently receive funding, and community-based for those who are re-applying. Any interested parties for the regional partnership grants should plan to attend the capacity-building portion.

For local in-person workshops, you will not receive a confirmation for registering – this mainly helps us for planning purposes in setting up the space.

2025-2027 Health System Change Grant

2025-2027 Health System Change grant is live.

The Indiana Department of Health’s (IDOH) Tobacco Prevention and Cessation (TPC) division supports the advancement of commercial tobacco treatment efforts through the funding of health systems change practices. The Health Systems Change partnerships (HSC) grant supports health systems to implement, maintain and expand tobacco and nicotine dependence treatment. TPC seeks partnerships with health systems providing inpatient care and outpatient care, including primary and/or specialty care, federally qualified health centers, community health centers, behavioral health, and dental and vision centers.

Additional partnerships may include organizations providing technical assistance, training, quality improvement and evaluation support to the health care settings in implementation of these strategies.
