- Are City employees offered Behavior Health / Employee Assistance Program benefits?
- Employees enrolled in the City’s group health insurance plan through UMR have access to mental health services provided by licensed professionals. These services are covered at 100% after the annual deductible is met ($3,400 for individuals or $6,800 for families).
- Public Safety employees only: Dr. Dardeen at Suncrest Psychological provides specialized psychological services. These services are billed confidentially and covered under the $4,000 budget allocated to the Public Safety Assistance Program (LIT 294 4-1020-431-150).
- Is it important to report a change of address to the HR?
Yes. It is important to keep your address current with the City of Logansport. Accurate information ensures proper communication and is necessary for insurance reporting, COBRA notifications, tax documents (including W-2 forms), and the Public Employees Retirement Fund. A Contact Information Change Form is available at the office of Human Resources.
- When should an employee report a work-related injury?
All work-related accidents and injuries must be reported within 24 hours to qualify for workers’ compensation coverage. This requirement applies to all injuries, regardless of severity, including those requiring only first aid.
- Can Human Resources help me understand my medical billing questions?
Yes. Medical billing and Explanation of Benefits (EOB) statements can be complex. Human Resources can assist employees in understanding their benefits and help address or resolve medical billing questions.
- What purchases can be made with an HSA account?
Funds in a Health Savings Account (HSA) can be used for qualified medical expenses as defined by the IRS. These include medical care, behavioral health services, vision and dental expenses, prescription medications, and certain long-term care services and insurance.
