Morgan County Maternal and Child Health Community Action Team

Core Services

Maternal and Child Health is a core public health service of the Health Department and has been identified by the Indiana Department of Health as one of the state’s five Core Public Health Services priorities. Two key performance indicators (KPIs) have been established to measure progress and guide improvement efforts:

KPI: Number of counties with documented processes to refer families to needed services including contraceptive care, WIC, home visiting, prenatal care, substance use disorder treatment, and insurance navigation

KPI: Number of counties that identified an opportunity to improve birth outcomes and implemented an evidence-based or promising program or activity to improve that birth outcome

To meet these KPIs, the Health Department established a Maternal and Child Health Collaboration at the end of 2025. The collaboration has since taken on the role of the Community Action Team, connected to the Fetal and Infant Mortality Review Board (FIMR) and Child Fatality Review Board (CFR).

To improve the health and well-being of mothers, infants, children, and families in Morgan County by building strong community partnerships, promoting equitable access to quality care and resources, engaging families, and advancing evidence-based strategies that foster healthy, thriving communities.

About

The collaboration consists of nearly 30 members representing 16 organizations. Our collaborative brings organizations together to reduce health inequities and help ensure all families can access the care, services, and resources they need regardless of where they live, income level, or background.

By partnering across agencies and community organizations, we work to improve coordination of services for families who often navigate multiple systems at once. Through shared communication, stronger referral networks, and coordinated support, we help reduce barriers to care and make services easier to access.

The collaboration also strengthens community trust and engagement by bringing together parents, caregivers, healthcare providers, schools, faith-based organizations, mental health professionals, and community partners. By including community voices in planning and decision-making, we help ensure programs and initiatives reflect the real needs of local families.
