The Morgan County Health Department can now issue birth and death certificates for any county in Indiana as long as the record is complete. Individuals born outside of Morgan County are advised to call ahead to verify before submitting an application: 765-342-6621.
Vital Records may only be obtained by certain individuals:
Birth Records
- Birth Certificate Application (to print and mail)
- Birth Certificate Application Form (to complete online and submit payment online via IN.gov)
Death Records
- Death Certificate Application (to print and mail)
- Death Certificate Application Form (to complete online and submit payment online via IN.gov)
