Indiana requires that all students have specific vaccinations prior to attending school to minimize student exposure to preventable, life-threatening disease (IC 20-34-4-2). If your child is behind on their immunizations, the health center will work with you and your child's provider to ensure he/she is caught up.
CHIRP (Children & Hoosier Immunization Registry Program) is a secure web-based application that is administered by the Indiana State Health Department and provides a database for student immunizations. ISBVI can use this registry to review vaccination records, therefore, please ensure your child's most recent vaccinations have been uploaded to CHIRP by his/her provider.
In the case of medical exemption, see the form listed below. In case of religious exemption, please contact the ISBVI Health Center for the appropriate form.
If you have further questions about immunizations, please review the Indiana State Department of Health website (https://www.in.gov/isdh/17094.htm) and the Center for Disease Control (CDC) website (https://www.cdc.gov/vaccines/schedules/index.html)
