Make time for your health - Schedule your annual physical today

It's easy to get caught up in the day-to-day grind and put our health on hold. But the truth is, our health is the foundation for everything else in our lives. That’s why completing an annual physical earns the highest reward in the Wellness Rewards Program—$200 for each employee and spouse covered on the state employee health plan.

During an annual physical, your Primary Care Provider (PCP) will collect measurements such as blood pressure, cholesterol, blood sugar levels and family history. These measurements provide valuable information about your current health and can help identify potential health risks.

Annual physicals at an in-network provider are covered 100%* by the state health plan for eligible employees and spouses, AND you can also earn $200 in Wellness Rewards for completing the appointment and submitting the required form.

How to find a PCP

3 ways to find a doctor in your plan:

  1. Log in at anthem.com and choose Find Care.
  2. Download the Sydney Health mobile app, log in, and select Find Care.
  3. Call Member Services at the number on your ID card.

How to complete your Annual Physical & earn a $200 Wellness Reward

  1. Make an appointment with your PCP for a physical with lab work.
  2. Print the Annual Physical Results form and fill in Section 1. Bring the form to your appointment and have your doctor complete Section 2.
  3. You are responsible for submitting your completed form. Follow instructions on the form to submit it via fax or upload it. Please note, it can take up to 4 weeks for the forms to be processed. Your form is considered processed when the ActiveHealth portal Rewards Center shows the activity as complete.

    Note: Results must be visible in the ActiveHealth portal by December 31, 2025, to earn the gift card.

*If non-preventive care is provided at the same time as the annual physical, there could be out-of-pocket costs.
