Hearing and Vision Screening

Hearing Screening

Hearing screening is required for all accredited public and accredited non-public schools for grades 1, 4, 7 and 10; as well as students transferring into the school and any student suspected of having a hearing defect. All accredited schools are required to report hearing screening results to the Indiana Department of Education via the School Health Report which is due by June 15 of each school year.

Vision Screening

Vision screening is required for all accredited public and accredited non-public schools for grades K or 1, 3, 5 and 8; as well as any student suspected of having a visual defect.  All accredited schools are required to report their vision screening results to the Indiana Department of Education via the School Health Report which is due by June 15 of each school year.

School Health Report

This report collects data regarding vision, hearing, and immunization information for students in grades K–12. Each building must submit a School Health Report. These reports cannot be combined and submitted as a corporation but must be done at the building level.

To complete this report, school personnel will need to access the IDOE data reporting website called “DOE Online.” To access this site and input your data you will need to have a password.

You may obtain your password at any time, but please note that the report will not open until March 1. The report will be open for several months (March to June) and schools are able to revise or edit their data as many times as needed until the report closes.
