The Opioid Treatment Program (OTP) Drive-Time Analysis dashboard displays the locations of all OTP locations across Indiana along with the drive-time for Hoosiers to access the OTP.

How to become an OTP Provider in Indiana
Indiana only allows certified hospitals, state psychiatric institutions, or Community Mental Health Centers (CMHCs) to open new OTPs. According to Indiana Code 12-23-18-5.5; Indiana is prohibited from approving new Independent OTP providers.
Eligible providers must complete:
- SAMSHA Certification Submit form SMA-162 to SAMSHA
- DEA Registration Submit Form 353 to DEA
- Indiana Controlled Substance Registration Apply through PLA
- DMHA OTP Licensure Submit via DMHA Provider Portal
- Accreditation Receive accreditation from a SAMSHA approved accrediting body
