INDIANA VETERAN BENEFIT APPLICATIONS:

• Certificate of Eligibility – Disabled Veteran Tax Deduction - State Form 51186 -WILL NO LONGER BE USED.  You will only need the 12662 shown below.
• Application for  Tax Deduction for Disabled Veteran and Surviving Spouses of Certain Veterans -12662  
  If we need to complete Section VIII on this form, you NEED to send us your DD214 and a VA award letter showing your combined percentage.  SSNs are not required on this form so we cannot look up or request verification of the veteran's service dates and disability percentage in the VA System. We will have to call you to get this information.
  If you have your annual tax abatement letter issued by the U.S. Department of Veterans Affairs, you may go directly to the County Auditor with the 12662.  We do not need to sign Section VIII. This letter shows your service dates, discharge type, and total percentage awarded by the VA. Additionally, the second page of this award letter lists wartime dates.   

If you need IDVA to verify service dates, discharge, and VA disability rating in Section VIII, send your completed 12662, discharge, and VA letter to:
