All applications must be accompanied by:

            1.  Approval request letter on letterhead.  Letter must be signed and dated on by someone in    

                 authority at the institution to become approved. 

            2.  Memo on letterhead signed and dated stating:  “I certify the information provided is true and correct

                 in content and policy.”

            3.  Three copies of current catalog/bulletin/brochures with attachments. 

            4.  VA Form 10-0144A State of Assurance of Compliance with Equal Opportunity Laws.
