Wage Statement Requests

The Indiana Department of Revenue can provide customers or their legal representatives with an income tax withholding statement. Requesters must be the taxpayer, taxpayer’s spouse who filed jointly, a fiduciary (administrator, executor or trustee), or other agent with power of attorney.

If the information is blank or incorrect

DOR can only provide withholding information supplied by the employer. Any change or updates to Form W-2 or W-2C may be missing. If there is missing information, you must contact the employer to request it. It is their responsibility to provide you with corrected information.

If you have any questions, message us through your INTIME account or call us at 317-232-2240 between 8 a.m. and 4:30 p.m. Monday through Friday.

Request by mail

To request the withholding information, you must include the taxpayer’s name, proof of your relationship to the taxpayer, the Taxpayer Identification Number or Social Security number, and the tax years needed. The requester must also sign the letter.

You may also fax the letter to 317-232-1843 or request the information at one of our District Offices. You must bring a letter and a valid photo ID.

Your statement will be mailed to our address on file unless you include a request for a change of address.
