This table is the IDHS schedule of fines and civil penalties that apply to violations of laws, rules and requirements of federal programs administered by the agency.

| IC 16-31-3-14(b) | Violations by EMS certificate/license holders | Up to $500 per violation per day | For acts of renewal dishonesty and late applications by provider organizations, fines are applied as identified in Note 1 and Note 2. For all other instances, fines are stayed until rules are drafted. |
