Obtain Death Certificate
To obtain a death certificate, you must have a vested interest in the record which would include power of attorney for the estate of the deceased, etc. If you were not married to the deceased at the time of death, you can not obtain a death certificate.
You must have the proper identification to obtain a death certificate. Our agency prefers a driver's license or state ID.
The death certificate fee is $13.00.
To obtain a death certificate by mail, you may complete the Application for a Certified Copy of a Death Record form and mail it with the required documentation and payment to the Health Department at 325 S. Oak Street, Suite 202, Winchester, IN 47394.
