If you've been selected for jury duty please fill out the forms (that were mailed to you) and mail them back as soon as possible in the envelope provided. IT'S VERY IMPORTANT THAT YOU MAIL THE FORMS BACK TO THE CLERK'S OFFICE TO KEEP THEM ON RECORD.
If you no longer live in this county, please fill out the forms and note that you no longer live in this county AND MAIL IT BACK TO THE CLERK'S OFFICE.
If you are over the age of 75 and would like to be taken off of the jury, please fill out the forms and request to be deferred for 12 months (see the bottom of the QUALIFICATION FORM) and mail them back in the envelope.
If you are sick or have an ailment please fill out the forms and note your ailment on #15 of the JURY QUESTIONNAIRE and request to be deferred for 12 months on the JUROR QUALIFICATION FORM (if you desire to be deferred).
If your spouse has passed away and they receive a jury duty notice please fill out their name and date of birth at the top of the JURY QUESTIONNAIRE, make a note at the bottom of the page stating they have passed away and sign your name and relationship (to the juror) at the bottom of the page (and mail back to the Clerk's Office).
If you request to be deferred from jury duty please know that the forms you return to us now will be looked at by the court at the time you are picked for an actual jury. NOT RETURNING YOUR FORMS WOULD ONLY DELAY THE DEFERRAL PROCESS.
