How to Apply

Please come into our office and meet with a caseworker to go over the application and list of documents that will need to be submitted before you can schedule an appointment. We are located at 1010 South Walnut Street Suite A, Bloomington, IN 47401. If you are unable to come into our office we have a printable version of both documents below. Please print the documents and call our office at (812) 336-3713 to speak with a case investigator. Please do not sign the application. You will do that during your interview in the presence of your township case investigator.

Public Notice

The Perry Township Board will meet at 11:00 am, Thursday, April 2, 2026 at the Perry Township Office, located at 1010 South Walnut Street, Suite D, Bloomington, Indiana, 47401. The purpose of the meeting is to discuss:
