- Proof of Income for the Last 30 days by providing:
- Bank Statements
- Award Letters from SS or SSI
- Check Stubs or Printout from Employer
- Support Printout or a Note from Payor with Dollar Amount, Time Period, Signature, Address, and Phone Number
- TANF Printout
- Unemployment Printout
