Our staff strives to assist our clients in their time of need with compassion and understanding. CHECKLIST FOR BLACK TOWNSHIP ASSISTANCE – PLEASE READ!
We need all applicable documents for you and all household members to complete this application and determine eligibility. Please review the list carefully to ensure that all required documents are returned to the Black Township Trustee office to avoid delays in your case.
A COMPLETED APPLICATION CONSISTS OF THE FOLLOWING DOCUMENTS THAT APPLY TO YOU:
- Application (Physical application must be returned to the Trustee’s Office)
- IDENTIFICATION:
- Valid Indiana ID (If 18 or over)
- Social Security Card
- BILLS:
- Lease Agreement, Mortgage Payment Information, Section 8 documents
- Electric Bill dated within the last 30 days
- Water Bill dated within the last 30 days
- Phone/Internet/Cable Bill dated within the last 30 days
- Any additional payments made within the last 30 days (ex: car payment, insurance, furniture payment, medical bills, credit card payment, etc)
- INCOME:
- Tax Returns
- Child Support documentation
- Pay Stubs for last 30 days
- Social Security Award Letter
- Section 8 Benefit Letter
- TANIF or Food Stamps Award Letter
- FINANCIALS:
- Last 30 days of bank statements (checking and savings)
- Last 30 days of activity if using a direct deposit pay card
- Release of Information, If 18 or over (Included in this packet)
- If you are needing assistance with rent, your office manager or landlord will need to fill out the rent verification form included in this packet.
*These documents are a part of the application process and will need to be returned to the Black Township Trustee office before we can review your case. Once you have all applicable documents submitted to us you will need to call the office to set your interview appointment.
