Chiropractors Licensing Information

Application Instructions

  • Chiropractor by Examination

    Examination: Applicants who have taken and passed Parts I, II, III, IV and Physiotherapy of the National Board of Chiropractic Examiners examination and have not been licensed to practice chiropractic in another state for at least three (3) years may apply under the examination criteria.

    1. Completed Application - Applications may be submitted online at MyLicense.IN.gov.
    2. Application fee of $100.00 - Pay by credit or debit card for applications submitted online. All application fees are nonrefundable.
    3. Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s). Please upload at the time of application or log back into your account and use the License Update option.
    4. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.  Please upload at the time of application or log back into your account and use the License Update option.
    5. Criminal Background Check
    6. Official Pre-Chiropractic Transcripts -  Applicants must submit official pre-chiropractic transcripts verifying completion of at least sixty (60) semester hours completed prior to chiropractic school.  Please upload at the time of application or log back into your account and use the License Update option.
    7. Official Chiropractic Transcripts -  Applicants must submit official chiropractic transcripts certifying receipt of a professional chiropractic degree.  Please upload at the time of application or log back into your account and use the License Update option.
    8. National Board of Chiropractic Examiners (NBCE) Scores -  Applicants must submit an official score report from the National Board of Chiropractic Examiners (NBCE), sent directly to the Board from the National Boards, showing completion of Parts I, II, III and IV with passing scores in all subjects, including Physiotherapy.       • Not Taken Part III of The NBCE Examination: Applicants who have taken the NBCE examination prior to September 1, 1987, are not required to submit Part III scores.
      • Not Taken Part IV of the NBCE Examination: Endorsement applicants who have not taken and passed Part IV of the NBCE examination are required to submit verification of a state constructed oral-practical examination, which is equivalent to Part IV of the NBCE examination, administered by the state where the applicant holds an active chiropractic license. Scores must be reported with the specific subject(s) and grade(s) obtained.
    9. Jurisprudence Examination -  All applicants for chiropractic licensure are required to pass a jurisprudence examination. No applicant is exempt from this requirement.
      • After the approval of your application by the Board, you will be notified by email that you are eligible to take the jurisprudence examination. Upon notification from the applicant that they are ready to take the jurisprudence examination, the examination and instructions will be emailed to you. You will have fourteen (14) days from the date the email is sent to you with the Jurisprudence Examination and Instructions to complete the examination and return the required information to our office.
      • All applicants will be examined on the statutes and rules of Indiana related to the practice of chiropractic. This is a 30 question, true-false, and multiple-choice examination. Passing criteria is 75%. The Jurisprudence examination is based on the Chiropractic Statutes and Administrative Rules available on the Resources page.

    Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.

    Abandon Applications:  If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.

  • Information Regarding Reciprocity

    Licensure by Reciprocity: The Committee shall issue a license to an applicant if the applicant satisfies the following conditions:

    • Holds a current license from another state or jurisdiction; and
      • that state's or jurisdiction's requirements for a license are substantially equivalent to or exceed the requirements for a license of the Committee; or
      • when the person was licensed or certified by another state:
        • there were minimum education requirements in the other state or jurisdiction;
        • if there were applicable work experience and clinical supervision requirements in effect, the person met those requirements to be licensed in that state; and
        • if required by the other state or jurisdiction, the person previously passed an examination required for the license or certification.
    • Has not committed any act in any state or jurisdiction that would have constituted grounds for refusal, suspension, or revocation of a license, certificate, registration, or permit to practice that occupation in Indiana at the time the act was committed.
    • Does not have a complaint or an investigation pending before the regulating agency in another state or jurisdiction that relates to unprofessional conduct.
    • Is in good standing and has not been disciplined by the agency that has authority to issue the license or certification.
    • If a law regulating the applicant's occupation requires the Committee to administer an examination on the relevant laws of Indiana, the Committee may require the applicant to take and pass an examination specific to the laws of Indiana.
    • Pays any fees required by the Committee for which the applicant is seeking licensure.
  • Chiropractor Limited Temporary Permit

    Basis for Limited Temporary Permit: A limited temporary permit issued under this section shall be limited to a specific activity, function, series of events, or purpose and to a specific geographical area within the state, which limitations shall be stated on the face of the temporary permit.

  • Chiropractic Graduate Student Permit
    1. Completed Application: Applications may be submitted online at MyLicense.IN.gov.  Please note the supervisor information on the paper form must be completed and signed by the supervisor.  The online application process requires the upload of the completed paper form.
    2. Certification from the Chiropractic School: Proof that the applicant is enrolled in the final year of course work by submission of an original school transcript or an original letter from the dean of the school.  The proof must be submitted directly to the Board from the school.  Please upload at the time of application or log back into your account and use the License Update option.

  • Continuing Education Provider
    1. Application - Apply online at https://mylicense.in.gov/eGov/ML1PLA.html
    2. Required Documentation - The following is required to be on the application or on documentation attached to the application:
      • Program Title - List the title of the program with each program date and location.
      • Continuing Education Hours Requested - The sponsor is required to break down the program submitted into the proper categories with the number of continuing education hours requested for each category.
      • Name of Instructor(s) - List the names of instructors on the application. Attach curriculum vitas or resumes.
      • Sample CE Certificate

    Sponsoring organizations are required to submit applications to the Board sixty (60) days prior to the presentation of the program.

Renewal Instructions

Chiropractor Licenses expire on July 1 of even-numbered years.

  • Active Renewal Information

    Renew Online!

    Continuing Education
    Continuing Education is required to renew your license. Information and requirements are available at the Resources page.

    Renewal Information
    Renewal notices are sent approximately ninety (90) days prior to the expiration date.  Licensees with a valid email address on file will be emailed the renewal information.  Licensees that do not have a valid email address on file will be mailed license renewal information.  This notice is mailed to the licensee's address of record with the Board.

    License Cards
    You can determine if your renewal has been received and processed by accessing our free Online Verification Search.  Please allow 24 hours (1 business day) for your license to be updated.  Once you verify that your license has been renewed, you may print a new license online in MyLicense One.

    Failure to Renew
    Failure to renew a license on or before the expiration date automatically renders the license invalid without any action by the Board.

    Renewal Fee
    The renewal fee is $100.00 prior to the expiration date.

  • Inactive Renewal Information

    A chiropractor may request to be placed on "Inactive Status" at the time of renewal by submitting the following:

    • Completion of a renewal application at MyLicense One.
    • Payment of the inactive renewal fee - $50.00

    The chiropractor is not required to complete continuing education to be placed on inactive status. The board may reinstate an inactive license upon notification and receipt of the following documentation:

    • A completed Activation Form.
    • Payment of the current renewal fee ($100.00).
    • Payment of the current reinstatement fee ($15.00).
    • Evidence of attendance of one (1) educational conference approved by the board for each year or portion of a year of inactive license classification.
  • Expired Renewal Information

    Expired less than 3 years

    Licenses that are expired less than 3 years may renew online using MyLicense One and will incur the late fee.

    Penalty Fees
    A licensee may renew a license up to three (3) years after the expiration date by paying a late fee of $50.00 in addition to the renewal fee of $100.00. A licensee who is currently on inactive/expired status may renew a license to an inactive status after the expiration date by paying a late fee of $50.00 in addition to the inactive renewal fee of $50.00.

    Expired Three (3) Years or more

    If your license has been expired for three or more years, you must reinstate your license to practice.  Please submit your reinstatement online with the additional documentation:

    • Reinstate Online!
    • Payment of the current renewal fee ($100) and current initial application fee ($100).  Total reinstatement fee:  $200.
    • Documentation to Upload:
      • Completion of continuing education for the time period the license has been expired.  The licensee will be required to provide copies of their continuing education certificates.

    Upon receipt of the above items you will be scheduled to appear before the Board at their next scheduled meeting.   The Board will determine if you will need to complete such remediation and/or additional training as deemed appropriate by the Board given the lapse of time involved.

  • Activation and Reinstatement Documents

    Print and mail the completed document with the required fee to the Indiana Professional Licensing Agency.  The address is on the document and checks should be made out to 'Indiana Professional Licensing Agency'.  If you answer 'YES' to any questions be sure to include a detailed response with your renewal.

Additional Information

  • Issuance of License/Permit

    Upon issuance of your Chiropractic license/permit by the Board, you will be sent an email notifying you that your license or temporary permit has been issued. There will be instructions on how to purchase a blue license card to be mailed to you or how to download a free license card for immediate printing.  The PLA Online Services page has a full list of options we offer online.

  • License Expiration and Continuing Education

    All chiropractic licenses expire on July 1 of even numbered years. Practitioners are required to have completed twenty- four (24) hours of continuing education during the renewal period which include eight (8) hours of courses in public health and/or risk management.

    • Continuing education is not required for the year in which the initial license was issued. Therefore, a person who was issued an original chiropractic license between the date of July 1 of an even numbered year and July 1 of an odd numbered year is only required to submit twelve (12) hours of continuing education including four (4) hours of public health and/or risk management courses. A person who is issued an original license after July 1 of an odd numbered year is not required to submit continuing education for the first renewal.

    Temporary Permit Expiration:  A temporary permit is valid for a nonrenewable period of not more than thirty (30) days and shall be limited to a specific activity, function, series of events, or purpose and to a specific geographical area within the state, which limitations shall be stated on the face of the temporary permit.

  • Chiropractic Graduate Student Additional Information
    Definitions:
    • “Graduate Student” means a student in the final year of course work at a chiropractic school or college.
    • “Supervisor” is a chiropractor licensed in the state of Indiana who acts as the direct supervisor and overseer of the educational process for the graduate student.
    Duties of the Supervisor:
    1. The supervisor of a graduate student shall hold an Indiana chiropractic license which is current and in good standing.
    2. A supervisor shall supervise no more than one (1) graduate student at any given time.
    3. The supervisor shall develop a training schedule in coordination with the school or college of chiropractic that will be followed by the graduate student throughout the education process.
    4. Chiropractors who supervise graduate students shall be exclusively responsible for the direct supervision of the graduate student.
    5. Upon successful completion of the educational program, the supervisor shall provide the Board with a letter stating the graduate student has completed a program from beginning date to ending date.
    Duties of a Graduate Student:
    1. A graduate student shall not provide an independent diagnosis of a patient.
    2. A graduate student will be required to maintain a log of chiropractic procedures that shall be reviewed daily by the supervisor and shall be available for review by the Board at the Board’s request.

    Approved Organizations

    Under Ind. Code 25-1-4-0.2, if a program is designed to directly enhance the chiropractor’s knowledge and skill in providing services relevant to the chiropractor’s profession or occupation and falls under an “Approved Organization,” it is not necessary to submit an application for approval by the Board of Chiropractic Examiners.

    Ind. Code 25-1-4-0.2:
    Sec. 0.2. As used in this chapter, “approved organization” refers to the following:

    • (1) United States Department of Education.
    • (2) Council on Post-Secondary Education.
    • (3) Joint Commission on Accreditation of Hospitals.
    • (4) Joint Commission on Healthcare Organizations.
    • (5) Federal, state, and local government agencies.
    • (6) A college or other teaching institution accredited by the United States Department of Education or the Council on Post-Secondary Education.
    • (7) A national organization of practitioners whose members practicing in Indiana are subject to regulation by a board or agency regulating a profession or occupation under this title or IC 15.
    • (8) A national, state, district, or local organization that operates as an affiliated entity under the approval of any organization listed in subdivisions (1) and (7).
    • (9) An internship or a residency program conducted in a hospital that has been approved by an organization listed in subdivisions (1) through (7).
    • (10) Any other organization or individual approved by the Board.

    Also, Ind. Code 25-1-4-0.5:

    • Sec. 0.5. As used in this chapter, “continuing education means an orderly process of instruction that is approved by an approved organization or the board that is designed to directly enhance the practitioner’s knowledge and skill in providing services relevant to the practitioner’s profession or occupation.
    Deadline for Submission of Application

    If your program does not fall under the statutory provision as listed above, the Sponsoring Organizations are required to submit applications to the Board sixty (60) days prior to the presentation of the program.

    Continuing Education Hours Not Acceptable

    According to 846 IAC 1-8-4(b)(2) Practice management courses will not be approved by the Board for continuing education.

    Risk Management

    Risk management is defined as the chiropractor’s legal responsibility to his or her patient.

    Sponsors

    Seminars must be sponsored by a college of chiropractic or an established chiropractic organization.

    Standards for Approval

    The Board will approve a course if it determines that the course will make a significant contribution to the professional competency of chiropractors who enroll. In determining if a course meets this standard, the Board will consider whether:

    • The course has substantial content.
    • The course content directly relates to the professional practice of chiropractic.
    • Practice management courses will not be approved by the board.
    • Each lecturer who has teaching responsibility in the course is qualified by academic work or practical experience to teach the assigned subject.
    • The course is of sufficient length to provide a substantial educational experience.
    • Courses of less than one (1) hour will be reviewed carefully to determine if they furnish a substantial educational experience.
    Approval Certificates

    Upon approval by the Board, a certificate will be issued and mailed to the sponsor.

    Record of Attendance

    As a condition of approval of programs, the sponsor must agree to provide participants with a record of attendance and to retain records of attendance by participants for four (4) years from the date of the program. The record of attendance presented to the chiropractor must state the sponsor, name of program, date of program, location of the program and the hours granted.
