Chiropractors Licensing Information

Application Instructions

  • General Information

    The Fair Information Practice Act: In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.

    Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.

    Abandon Applications: If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.

  • Chiropractor by Examination

    Examination: Applicants who have taken and passed Parts I, II, III, IV and Physiotherapy of the National Board of Chiropractic Examiners examination and have not been licensed to practice chiropractic in another state for at least three (3) years may apply under the examination criteria.

    Submit the following with your online application:

    1. Application fee of $100.00: Pay by credit or debit card. All application fees are nonrefundable.
    2. Criminal Background Check
    3. Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
    4. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    5. Official Pre-Chiropractic Transcripts: Applicants must submit official pre-chiropractic transcripts verifying completion of at least sixty (60) semester hours completed prior to chiropractic school.
    6. Official Chiropractic Transcripts:   Applicants must submit official chiropractic transcripts certifying receipt of a professional chiropractic degree.
    7. National Board of Chiropractic Examiners (NBCE) Scores: Applicants must submit an official score report from the National Board of Chiropractic Examiners (NBCE), sent directly to the Board from the National Boards, showing completion of Parts I, II, III and IV with passing scores in all subjects, including Physiotherapy.       • Not Taken Part III of The NBCE Examination: Applicants who have taken the NBCE examination prior to September 1, 1987, are not required to submit Part III scores.
      • Not Taken Part IV of the NBCE Examination: Endorsement applicants who have not taken and passed Part IV of the NBCE examination are required to submit verification of a state constructed oral-practical examination, which is equivalent to Part IV of the NBCE examination, administered by the state where the applicant holds an active chiropractic license. Scores must be reported with the specific subject(s) and grade(s) obtained.
    8. Jurisprudence Examination:  All applicants for chiropractic licensure are required to pass a jurisprudence examination. No applicant is exempt from this requirement.
      • After the approval of your application by the Board, you will be notified by email that you are eligible to take the jurisprudence examination. Upon notification from the applicant that they are ready to take the jurisprudence examination, the examination and instructions will be emailed to you. You will have fourteen (14) days from the date the email is sent to you with the Jurisprudence Examination and Instructions to complete the examination and return the required information to our office.
      • All applicants will be examined on the statutes and rules of Indiana related to the practice of chiropractic. This is a 30 question, true-false, and multiple-choice examination. Passing criteria is 75%. The Jurisprudence examination is based on the Chiropractic Statutes and Administrative Rules available on the Chiropractor Home page.
  • Information Regarding Reciprocity

    Licensure by Reciprocity: The Committee shall issue a license to an applicant if the applicant satisfies the following conditions:

    • Holds a current license from another state or jurisdiction; and
      • that state's or jurisdiction's requirements for a license are substantially equivalent to or exceed the requirements for a license of the Committee; or
      • when the person was licensed or certified by another state:
        • there were minimum education requirements in the other state or jurisdiction;
        • if there were applicable work experience and clinical supervision requirements in effect, the person met those requirements to be licensed in that state; and
        • if required by the other state or jurisdiction, the person previously passed an examination required for the license or certification.
    • Has not committed any act in any state or jurisdiction that would have constituted grounds for refusal, suspension, or revocation of a license, certificate, registration, or permit to practice that occupation in Indiana at the time the act was committed.
    • Does not have a complaint or an investigation pending before the regulating agency in another state or jurisdiction that relates to unprofessional conduct.
    • Is in good standing and has not been disciplined by the agency that has authority to issue the license or certification.
    • If a law regulating the applicant's occupation requires the Committee to administer an examination on the relevant laws of Indiana, the Committee may require the applicant to take and pass an examination specific to the laws of Indiana.
    • Pays any fees required by the Committee for which the applicant is seeking licensure.
  • Chiropractor Limited Temporary Permit

    Basis for Limited Temporary Permit: A limited temporary permit issued under this section shall be limited to a specific activity, function, series of events, or purpose and to a specific geographical area within the state, which limitations shall be stated on the face of the temporary permit.

    Submit the following with your online application:

    1. Application fee of $50.00 - Pay by credit or debit card. All application fees are nonrefundable.
    2. Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s).
    3. Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
    4. Official Chiropractic Transcripts:  Applicants must submit official chiropractic transcripts certifying receipt of a professional chiropractic degree.
    5. Verification of State Licensure(s): Verification of any registration/license/certification to practice any health-related profession or occupation in another state or territory.  Verifications must be submitted directly from the state of issuance.

    A temporary permit is valid for a nonrenewable period of not more than thirty (30) days and shall be limited to a specific activity, function, series of events, or purpose and to a specific geographical area within the state, which limitations shall be stated on the face of the temporary permit.

  • Chiropractic Graduate Student Permit

    Submit the following with your online application:

    1. Supervisor Information: Please note the supervisor information on the paper form must be completed and signed by the supervisor.  The online application process requires the upload of the completed paper form.
    2. Certification from the Chiropractic School: Proof that the applicant is enrolled in the final year of course work by submission of an original school transcript or an original letter from the dean of the school.  The proof must be submitted directly to the Board from the school.  Please upload at the time of application.
  • Continuing Education Provider

    Submit the following with your online application:

    1. Program Title: List the title of the program with each program date and location.
    2. Continuing Education Hours Requested: The sponsor is required to break down the program submitted into the proper categories with the number of continuing education hours requested for each category.
    3. Name of Instructor(s): List the names of instructors on the application. Attach curriculum vitas or resumes.
    4. Sample CE Certificate

    Sponsoring organizations are required to submit applications to the Board sixty (60) days prior to the presentation of the program.

Renewal Instructions

  • Renewal Information

    Renewal Information

    Chiropractor Licenses expire on July 1 of even-numbered years. Renewal notices are sent approximately ninety (90) days prior to the expiration date.  Licensees with a valid email address on file will be emailed the renewal information.  Licensees that do not have a valid email address on file will be mailed license renewal information.  This notice is mailed to the licensee's address of record with the Board.

    Failure to Renew

    Failure to renew a license on or before the expiration date automatically renders the license invalid without any action by the Board.

    Inactive Status

    A chiropractor may request to be placed on Inactive Status at the time of renewal.  The renewal fee will only be $50.00 for renewing to the Inactive status. The chiropractor is not required to complete continuing education to be placed on inactive status.

  • Activation Information

    Activation of Inactive License

    A chiropractor may request to activate their license from Inactive status while in the Inactive status by submitting the following:

    • In your MyLicense One account, select the Request Activation action item from your linked license
    • Payment of the Activation Fee of $115.00
    • Evidence of attendance of one (1) educational conference approved by the board for each year or portion of a year of inactive license classification.

    If your Inactive license has been expired less than 3 years, prior to submitting an Activation Request, you must first Renew your license to update the expiration date and then you may use the above Activation instructions.

  • Reinstatement Information

    Expired Three (3) Years or more

    If your license has been expired for three or more years, you must reinstate your license to practice.  Please submit your reinstatement online with the additional documentation:

    • Payment of the current renewal fee ($100) and current initial application fee ($100).  Total reinstatement fee:  $200.
    • Documentation to Upload:
      • Completion of continuing education for the time period the license has been expired.  The licensee will be required to provide copies of their continuing education certificates. You may email pla5@pla.in.gov to find out the total number of hours needed

    Upon receipt of the above items you will be scheduled to appear before the Board at their next scheduled meeting.   The Board will determine if you will need to complete such remediation and/or additional training as deemed appropriate by the Board given the lapse of time involved.

  • Continuing Education Information

    The Indiana Board of Chiropractic Examiners will accept any part or all of the required continuing education hours by distance learning that is provided by an approved organization as specified in Indiana Code IC 25-1-4-0.2.

    Practitioners are required to have completed twenty-four (24) hours of continuing education acquired after July 1, 2024.  Included within the twenty-four (24) hour requirement are eight (8) hours of approved courses in public health and/or risk management.

    Continuing education is not required for the year in which the initial license was issued. Therefore, a person who was issued an original chiropractic license between the dates of July 1, 2024 and July 1, 2025 is only required to submit twelve (12) hours of continuing education including four (4) hours of public health and/or risk management courses. A person who is issued an original license after July 1, 2025 is not required to submit continuing education for the next renewal.

    Approval of Chiropractic Continuing Education Programs

    Under the statutory provision as listed below, if a program is designed to directly enhance the practitioner's knowledge and skill in providing services relevant to the chiropractor's profession or occupation and falls under an "Approved Organization" it is no longer necessary to submit an application for continuing education approval by the Indiana Board of Chiropractic Examiners.

    Under IC 25-1-4-0.2:

    Sec. 0.2. As used in this chapter, "approved organization" refers to the following:

    1. United States Department of Education.
    2. Council on Post-Secondary Education.
    3. Joint Commission on Accreditation of Hospitals.
    4. Joint Commission on Healthcare Organizations.
    5. Federal, state, and local government agencies.
    6. A college or other teaching institution accredited by the United States Department of Education or the Council on Post-Secondary Education.
    7. A national organization of practitioners whose members practicing in Indiana are subject to regulation by a board or agency regulating a profession or occupation under this title or IC 15.
    8. A national, state, district, or local organization that operates as an affiliated entity under the approval of any organization listed in subdivisions (1) and (7).
    9. An internship or a residency program conducted in a hospital that has been approved by an organization listed in subdivisions (1) through (7).
    10. Any other organization or individual approved by the Board.

    Also, under IC 25-1-4-0.5:

    Sec. 0.5. As used in this chapter, "continuing education means an orderly process of instruction that is approved by an approved organization or the board that is designed to directly enhance the practitioner's knowledge and skill in providing services relevant to the practitioner's profession or occupation.

    All programs, which are sponsored by the above organizations, must directly enhance the practitioner's knowledge and skill in providing services relevant to the chiropractic profession.

    Licensee Responsibilities

    • Continuing education credits may not be carried over from one renewal period to the next renewal period.
    • A maximum of twelve (12) hours may be obtained within each licensure year.
    • A licensee must retain a record of completion of continuing education for three (3) years.
    • A licensee must present verification of completion of continuing education upon request by the Board.
    • All programs attended must directly enhance the practitioner's knowledge and skill in providing services relevant to the chiropractic profession or occupation.
    • All programs that are attended to renew your chiropractic license must fall under an approved organization as stated in IC 25-1-4-0.2 or be approved by the Board.
    • The Board will not approve or accept for renewal of licensure programs in the area of practice management.
    • Public Health and/or Risk management hours must be designated on your certificate or transcripts as public health and/or risk management hours or credit will not be given.
    • If a program does not fall under IC 25-1-4-0.2 as an Approved Organization, the Sponsoring Organization is required to submit an application for continuing education approval.

    Risk Management

    Risk management is defined as the chiropractor's legal responsibility to his or her patient as provided by 846 IAC 1-3-3 and IC 25-1-9.

    Continuing Education Certificates

    A licensee is not required to submit copies of their continuing education certificates at the time of renewal. By renewing online or signing the renewal application you swear or affirm under the penalties of perjury that you have completed the continuing education requirement. A practitioner shall retain copies of continuing education certificates for three (3) years from the end of the licensing period. After the renewal, a percentage of practitioners will be chosen in a random audit to submit copies of their continuing education certificates. If the practitioner does not comply with the audit the Board will proceed with action against the licensee.

    Continuing Education Audit

    After the conclusion of a renewal, a random selection of renewed licenses will be chosen to be audited.  The Board will then reach out to the participating licensees to submit their completed Chiropractic Audit Tracking Sheet and submit the copies of their certificates of completion for review.  Continuing education only needs to be submitted if requested by the Board.

Additional Information

  • Chiropractic Graduate Student

    Definitions:

    • “Graduate Student” means a student in the final year of course work at a chiropractic school or college.
    • “Supervisor” is a chiropractor licensed in the state of Indiana who acts as the direct supervisor and overseer of the educational process for the graduate student.

    Duties of the Supervisor:

    1. The supervisor of a graduate student shall hold an Indiana chiropractic license which is current and in good standing.
    2. A supervisor shall supervise no more than one (1) graduate student at any given time.
    3. The supervisor shall develop a training schedule in coordination with the school or college of chiropractic that will be followed by the graduate student throughout the education process.
    4. Chiropractors who supervise graduate students shall be exclusively responsible for the direct supervision of the graduate student.
    5. Upon successful completion of the educational program, the supervisor shall provide the Board with a letter stating the graduate student has completed a program from beginning date to ending date.

    Duties of a Graduate Student:

    1. A graduate student shall not provide an independent diagnosis of a patient.
    2. A graduate student will be required to maintain a log of chiropractic procedures that shall be reviewed daily by the supervisor and shall be available for review by the Board at the Board’s request.
  • Retirement

    Retirement from Practice

    Any chiropractor licensed to practice chiropractic in this state who intends to retire from practice shall notify the board in writing of the chiropractor's intention to retire and shall surrender the license to the board. Upon receipt of this notice and license, the board shall record the fact that the chiropractor is retired and excuse the person from further payment of license renewal fees and attendance at license renewal seminars. If any chiropractor surrenders the license to practice chiropractic in this state, the chiropractor's reinstatement may be considered by the board on the chiropractor's written request. If any disciplinary proceedings under this chapter are pending against a chiropractor, the chiropractor may not surrender the license without the written approval of the board.
