Required for obtaining a birth or death certificate:
- Completed application (obtained from the Health Department)
- Photo ID (for example, a drivers license, school ID or work ID)
- Birth certificates can be issued only to certificate holders, their spouse, mother, father, grandparent, children (over age 18), aunt, and uncle.
