Immunizations
The Howard County Health Department offers nearly all CDC required & recommended immunizations for all ages to all Howard County residents.
Scheduling an immunization appointment
Appointments are preferred for immunizations, but walk-ins are welcome. Most insurances as well as Medicaid are accepted. If you are uninsured or underinsured, please ask about pricing because you or your child may be eligible for vaccines at a reduced cost.

What to bring to the appointment
- Parent or guardian must be present for anyone under the age of 18 years old
- Immunization records (including out of state or country records)
- Insurance card
- If you are underinsured please inquire with office.
Immunization records
To request a copy of your immunizations record, please call 765-456-2408, option 1 for nursing. Alternatively, you may request a copy of your immunizations record from MyVaxIndiana.
Immunization Clinics
The Howard County Health Department is committed to supporting our community by providing access to immunizations for all residents, promoting individual and community wellness, and reducing the spread of vaccine-preventable diseases.
We offer on-site mobile vaccination services throughout Howard County! Our team provides the nursing staff, immunizations, and equipment needed to facilitate the clinic. Vaccines are available for all ages—infant, child, and adult.
To host a clinic, your organization will need to provide:
- 6–8 tables and chairs
- A large trash can
- Access to an electrical outlet for our mobile printer (used for shot records and appointment reminders)

If you complete the PDF form, please scan and email it to kelly.carpenter@howardcountyin.gov at least 4 weeks before your desired clinic date.
Note: Clinics are available Monday through Friday; weekend clinics are not available at this time.
