Death certificates for persons who were declared deceased in Indiana are available from the Health Department. Amendments are made through the county of death. We are not able to issue a death certificate for those who died in any other state. Please read the Birth and Death ID Requirements to see who qualifies to purchase a death certificate and what identification is required.
