About the Personal Care Services Agency (PSA) Licensing Program
The Indiana Department of Health (IDOH) licenses agencies that provide attendant care services; homemaker services that assist with or perform household tasks, including housekeeping, shopping, laundry, meal planning and preparation, and cleaning; and companion services that provide fellowship, care, and protection for a client, including transportation, letter writing, mail reading, and escort services.
These services are provided to a client at the client's residence by the staff of a licensed personal care services agency (PSA).

Policies are shown on page four of the application and are expected to be submitted in the order shown, along with any required forms and tests. Do not submit any policies other than those instructed; each policy is attributed to Indiana Code (IC) that describes the requirement of the policy. Please write your agency policies in accordance with the requirements for each statute.

- Verify that the application is signed and dated by the owner or manager of the agency.
- Click here to access the payment portal.
- Use your six-digit facility/license number for your reference number when you access the link.
- Email your payment receipt to PSA@health.in.gov.
- Email your renewal application to PSA@health.in.gov.

The licensing process described below will be implemented on May 1, 2026.
Gateway
The IDOH Gateway is used to complete initial registrations, renewals, upload required documents, and complete payments for PSAs. The IDOH Gateway is accessed through Access Indiana. Once access is gained to Access Indiana and the IDOH Gateway, users may request access to the Personal Service Agency License Registration application.
Register for Gateway
- Click here to access the IDOH Gateway.
- If you do not have an account, on the right-hand side, click Step 1 - New User Register with Access Indiana ("Register to Access Indiana").
- When prompted, select Personal Service Agency from the drop-down menu for applications.
- If you have an account but need application access, on the right-hand side, click Step 2 - Request for Gateway or New Application Access ("Request Access").
- When prompted, select Personal Service Agency from the drop-down menu for applications.
- If you have an account and have application access, on the right-hand side, click Step 3 - Registered Users with Gateway and Access Indiana ("Sign in with Access Indiana").
- If you do not have an account, on the right-hand side, click Step 1 - New User Register with Access Indiana ("Register to Access Indiana").
Once registration is completed successfully, users will receive a confirmation message.
Once an administrator approves the registration, users will receive an email notification of approval. The approval email will include a registration identification (ID) number that users will use to sign into the IDOH Gateway.
The licensing process described below will be implemented on May 1, 2026.
Initial Licensure
- Click here to log into the IDOH Gateway.
- On the right-hand side, click Step 3 - Registered Users with Gateway and Access Indiana ("Sign in with Access Indiana").
- Access the licensure application by clicking on the Personal Service Agency License Registration button.
- At the bottom left-hand corner of the page, click the Register New Agency button.
- Enter basic details of the agency; all fields are mandatory, including Agency Hours.
- When completed, click the Register button.
- The Details page is displayed, which includes payment amount and effective date of the license. Review and complete all details for the agency.
- When the data is entered (for Details and Ownership), click Next. The data entered is validated. If any corrections are required, they must be resolved before proceeding.
- Review the Ownership details and click on the Next button.
- Complete the Managers section. Click Next.
- Enter the Branch details.
- If the agency has multiple locations, click Add Location button to add additional locations.
- Upload required documents in Documents section.
- In the Attestation section, enter authorizing designee and complete the attestation. Click Submit button.
- Enter transaction details. Click Proceed.
- Enter payment details. Click Submit Payment.
- Once payment is processed, click Continue and a payment confirmation is displayed. An email confirmation is also sent to the email address on-file for the agency.
The licensing process described below will be implemented on May 1, 2026.
License Renewal
The option for renewal becomes available at the 60-day mark for the current expiration of each agency. For example, if a license expires on Dec. 15, 2025, the agency will be able to renew their registration no earlier than Oct. 16, 2025.

- On the right-hand side, click Step 3 - Registered Users with Gateway and Access Indiana ("Sign in with Access Indiana").
- Access the licensure application by clicking on the Personal Service Agency License Registration button.
- Search for agency by its employer ID number or by its name.
- When agency is located, click on View Details.
- At the bottom left-hand corner of the page, click the Renew button.
- Review and complete all details for the agency.
- Upload required documents in Documents section.
- In the Attestation section, enter authorizing designee and complete the attestation. Click Submit button.
- Enter transaction details. Click Proceed.
- Enter payment details. Click Submit Payment.
- Once payment is processed, click Continue and a payment confirmation is displayed. An email confirmation is also sent to the email address on-file for the agency.
Report a Complaint
Anyone may submit a complaint to IDOH about care provided at any licensed or certified Indiana healthcare facility or entity. IDOH is available to receive complaints against nursing homes, hospitals, home health agencies, hospice agencies, personal service agencies, and intermediate care facilities for persons with intellectual disabilities (ICF-IID; also called group homes). IDOH investigates all eligible complaints.
