Report a Complaint

Anyone may submit a complaint to IDOH about care provided at any licensed or certified Indiana healthcare facility or entity. IDOH investigates all complaints.

State or federal regulations require health facilities or entities to report certain types of “incidents” to IDOH.

Report a Reasonable Suspicion of a Crime

Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long-term care resident to IDOH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include nursing homes, hospice facilities, or intermediate care facilities for individuals with intellectual disabilities.

Survey Report System

The survey report system is utilized as a part of licensing and federal certification of healthcare facilities. The system enables facilities to view/print survey reports and survey-related documents online, allows facilities to complete the survey plan of correction, and allows facilities to upload supporting documents for a plan of correction for a survey.
