The Long-Term Care Newsletter provides program updates related to licensing and healthcare quality information for long-term care facilities.
Report a Complaint
Anyone may submit a complaint to IDOH about care provided at any licensed or certified Indiana healthcare facility or entity. IDOH investigates all complaints. This report link is to a page that provides information about the complaint process and how to report a complaint. A complaint report form is provided for individuals to assist in the reporting of care issues. |
Report an Incident
State or federal regulations require health facilities or entities to report certain types of “incidents” to IDOH. This report link is to a page that provides information about the incident reporting process and how a healthcare facility or entity reports incidents. |
Report a Reasonable Suspicion of a Crime
Federal law requires “covered individuals” to report a reasonable suspicion of a crime against a long-term care resident to IDOH and a local law enforcement entity. Covered individuals include an owner, operator, employee, manager, agent, or contractor of the facility. The included facilities include nursing homes, hospice facilities, or intermediate care facilities for individuals with intellectual disabilities. This report link is to a page that provides information for long-term care providers about the crime reporting requirements and how to report a reasonable suspicion of a crime against a resident. Long-Term Care Licensing and Certification Programs
Survey Report System
The survey report system is utilized as a part of licensing and federal certification of healthcare facilities. The system enables facilities to view/print survey reports and survey-related documents online, allows facilities to complete the survey plan of correction, and allows facilities to upload supporting documents for a plan of correction for a survey.
