COVID-19 Rental Assistance Program

Below are the required documents for the Covid-19 Rental Assistance Program. Please complete these documents and return them to the email address provided to you for your intake specialist. In addition, please provide a copy of the tenant’s current lease agreement. If the lease is month-to-month, please provide a copy of the original lease that shows a month-to-month clause. Please only return these documents for tenants specified in the email sent to you by IHCDA. If you have additional tenants who are applying, please wait to receive our email before sending the documents to the correct intake specialist.

*IHCDA will review the information on your completed W-9 to determine if your entity is required to receive a Form 1099-MISC.
