Fraud Reporting for Insurers & Producers

Effective July 1, 2026, IC 27-1-3-22(c) requires any person or entity regulated under Title 27 that has knowledge or a reasonable belief that a fraudulent insurance act is being or has been committed to furnish information to the IDOI or the National Insurance Crime Bureau (NICB) within 60 days of receiving notice.

 How to Report to IDOI

Send suspected insurance fraud information to: companyfraudreports@idoi.in.gov

When emailing, please include relevant dates, times, names, and a clear explanation of what is suspected.

DO NOT INCLUDE:

  • Social Security numbers,
  • PII/PHI, or
  • specific banking information.

If this information is necessary for the investigation, the IDOI will reach out with instructions for how to securely provide these details.
