Welcome to the home page for the Indiana Department of Health (IDOH) Division of Radiology and Weights & Measures. The division provides guidance and assistance to Indiana citizens and health care providers regarding radiation safety and protection. The division is responsible for the licensing and registration of medical radiology professionals and medical radiation sources in the State of Indiana. The division provides the following services:
- Licensing of radiology professionals, which include radiologic technologists, limited radiographers, nuclear medicine technologists, and radiation therapists
- Radiation machine registration and compliance
Announcements
It is an option for individuals to apply online for a first-time license/certificate in Indiana for seven categories after completing requirements. Please see details under Important Radiology Licensing Information.
Renew License/Certificate
To renew a license, we encourage everyone to renew their license online.
These are tips to renew a license/certificate and update contact information:

- Go to Personal Licensing and select “Login to Personal Licensing with Access Indiana”
- Select “Register for an Access Indiana account”
- There is a link for “Get help creating your account”, too
- On the web page “Licensing Home”, select “Renew License” on the left side under “Menu”
If you are not able to renew your license online, you may send your renewal form and payment to the address on the renewal form. Please contact our office at Radiology@health.IN.gov if you have two licenses to renew. After our division waives the renewal fee for one license, you may renew both licenses online.
Important Radiology Licensing Information
The Indiana Department of Health (IDOH), Division of Radiology and Weights & Measures, issues radiology permits, licenses, and certificates for eight categories. Indiana does not have reciprocity with other states. A permit, license or certificate from another state is not accepted. Permits are required to complete clinical training in Indiana. They are issued to individuals when they enroll in an approved radiography education program. Permits are not licenses. After individuals complete the approved education program, clinical training (with an active permit in Indiana), and pass an approved exam, they may apply for a license/certificate. To apply for a license for the first time in Indiana.
Individuals may apply online or by mail for a first-time license/certificate for:

Individuals may apply by mail for a Dental Radiographer license after completing requirements.
First Time Applicants
FIRST TIME ONLINE APPLICATION:
Individuals may apply online at this link: mylicense.in.gov.
- Login or create login credentials under Personal Licensing using the “Register for an Access Indiana Account” option.
- On the Licensing Home Page, go to the left side of the page under “Menu” and select “Initial Application”.
- The page “Application for Licensure” will appear.
- For “Profession”, select “Radiology”.
- For “License Type”, select the appropriate license.
- For “Obtained By Method”, select “Application”.

Documentation is required to apply for a first-time license/certificate for:
- Cardiac Catheterization Radiographer
- Chest Radiographer
- Chiropractic Radiographer
- Podiatric Radiographer
If applying online for a license/certificate for the above categories, the applicant must upload digital scans of these documents:
- Clinical training form (Application for Proficiency Certification for Limited Radiographer) completed by the program director of the approved radiography education program or the licensed practitioner, who oversaw the clinical training while the provisional permit was active
- Education diploma/certificate of completion from the approved radiography education program (links are below for IDOH-approved radiography educational programs by category)
- Proof of passing an approved exam

FIRST TIME MAIL APPLICATION (if not able to apply online):
To apply for a license/certificate by mail: complete, print, and mail the license application (Application for Radiology License) and a check/money order for $60 to the address on the license application.

First-Time Dental Radiographer Licenses
After individuals meet education, clinical training (with permit), and exam requirements, they would apply by mail for a license. There is no option to apply online at this time. Please complete, print, and mail the license application (Application for Radiology License) and a check/money order for $60 to the address on the license application. Also, applicants or program directors need to provide the clinical training form, education diploma/certificate of completion, and exam documents before the IDOH will process a license. Graduates of CODA dental programs will not need to provide the Proficiency Certification form. For questions about the documents, contact the program director of the approved radiography education program.

To verify an active license or permit: Go to “Search & Verify” at this link: https://mylicense.in.gov/EVerification/Search.aspx.
For license verification for a travel tech: If form required by another state, send verification form from the state to radiology@health.in.gov or fax it to 317-233-7154. The top portion is completed by the tech and the bottom portion is completed by IDOH.
For address change: If you have moved, please update your address. This will ensure that your license renewal reminder and license are delivered to you in a timely manner. You can change your address with these options: online at mylicense.in.gov, by email Radiology@health.IN.gov, fax to 317-233-7154 or mail to IDOH:

For email, fax and mail options, provide the following: Name, old address, new address, license number, last 4 digits of your Social Security Number, and date of birth. There is no fee for address changes.
For Name Change: If you need to change your name on your license, please send proof of your name change (copy of marriage license, specific pages of divorce decree, legal name change document) to the division at Radiology@health.IN.gov with your old name, new name, license number, last 4 digits of your Social Security Number, and date of birth. Your name will be changed in the database. You will receive a new license after you renew your license. Also, you may verify that you have a license in your current name by going to “Free Search & Verify” at this link. If you would like to receive a new license soon with your current name on it, there is a $20 fee. Please ask for a Name Change Form.

Reporting a Complaint
To file a complaint about an individual that is operating a radiation producing machine or a medical practitioner regarding their employment of a radiographer or the use of their radiation producing machine(s) mail complaint to the address or email below.
