Required Documents for Child Caring Institutions
The following instructions apply to child caring institutions that are licensed under 465 Indiana Administrative Code (IAC) 2-9, 2-10, or 2-11.
For any child caring institution that is classified as a group home with 10 or less children under 465 IAC 2-12 or 465 IAC 2-13, a plan submission is not required; please submit an application through the Indiana Department of Homeland Security's (IDHS) portal.
Each of the following documents must be transmitted via the state's secure File Transfer Protocol (FTP) client in one zipped file:
- Attestation
- Plans/Drawings
- Scope
- Specification Documents

Submission Instructions
Attestation
All submissions must include a completed Design Professional's and Licensee's Attestation (click here to download).
Plans/Drawings
All drawings (including site utilities for any project not previously licensed or submitted for approval) should be submitted electronically via the state's secure FTP client in Drawing Web Format (DWF) or DWFx format using this template.
The recommended FTP client is FileZilla; however, firms may use any client that will allow a successful transfer. FileZilla may be downloaded here.

Scope
A complete and detailed description of the use of the proposed institution scope must be submitted in PDF and include:
- A complete and detailed description of the use of the proposed institution and its prior use (if any)
- The rule under which the institution will be licensed
- The maximum number of children for whom services will be provided
- The total number of children's bedrooms
- Describe the proposed staffing model of the institution
- Will the institution be staffed by resident staff (staff who sleep in the home)?
- Will the institution be staffed with shift work (no staff will sleep in the home)?

Specification materials/manuals must be submitted in PDF.
