Portable X-ray Services (PXR) Program

Welcome to the portable X-ray (PXR) certification program home page. A PXR supplier is a mobile unit that is not permanently located in a nursing facility or hospital nor fixed at any one location. In Indiana medical radiology professionals are licensed and medical radiation sources (facility/base office and radiation machines) are registered by the Medical Radiology Services Program. Also, all radiation machines used in Indiana must be inspected by an individual approved by the Medical Radiology Services Program before the equipment may be used on patients in Indiana. The registration is separate and must be completed before PXR certification for Medicare/Medicaid can take place.

• Licensing of radiology professionals (to include radiologic technologists, limited radiographers, nuclear medicine technologists, and radiation therapists)
• Radiation machine registration and compliance

To be certified and reimbursed by Medicare/Medicaid a PXR must meet the federal Conditions for Coverage found at 42 Code of Federal Regulation (CFR) Part 486, Subparts A-C. A PXR must have a base office address. The supplier, its personnel and its equipment must be licensed or registered as appropriate by the Medical Radiology Services program. Equipment must meet applicable FDA standards and be registered by the Medical Radiology Services program. The performance of x-ray tests must be under the supervision of a physician. Finally, the PXR must have an onsite survey by surveyors from the PXR Certification Program and found to be in compliance with federal and state regulations before being certified.

Report a complaint regarding a health care facility
Individuals can call or email to make complaints about care provided at any licensed or certified Indiana health care providers or suppliers.

Report an incident regarding a health care facility
The Incident Report Form is for health care facilities to notify the Indiana Department of Health of a reportable incident pursuant to the IDOH Reportable Unusual Occurrence Policy.  The Incident Report Form is also for health care facility staff (nursing homes, intermediate care facilities, and hospice agencies) to report a reasonable suspicion of a crime against a resident pursuant to Federal regulations.  This form is not to be used to file a complaint.
