FSSA’S Life Experience Project

The Office of Healthy Opportunities is interested in learning about the lived experiences of FSSA staff. The Life Experience Project Q&A provides a platform for individuals from different backgrounds to share their lived experiences. The goal is to share these experiences with others and encourage understanding and empathy based on learning about shared and lived experiences of diverse people within the agency.

Ways to Participate

We are currently accepting responses until the end of the year. If you are interested, please follow the link to our online form or the accessible form for staff who are deaf or hard of hearing. Participants are encouraged to submit responses to any or all questions included in the Q&A. Please note:

  • Participation is voluntary.
  • You can also participate anonymously!
    • Our team will not share any directly identifying information (e.g., names, addresses, divisions).
  • An edited version of your experience might be shared on the OHO SharePoint site.
    • Before your profile is shared, someone from our team will work you to with edit your responses for clarity and grammar.
  • Not all submissions will be shared publicly.
    • All responses will help the Office of Health Opportunities further tailor tools developed to improve staff experience across FSSA.
  • As the project develops, there may be the opportunity to also participate in video interviews. Please provide your contact information at the end of the survey if you are interested in next steps.
  • We will need permission to use and reproduce your responses for any potential promotional purposes. You can visit the Life Experience Project Q&A page for more information.
