Deaf/Hard of Hearing Community Needs Survey

Deaf and Hard of Hearing Services is doing a public assessment of the deaf and hard of hearing community to identify areas for improvement in Indiana. With this data, Hoosiers can assist DHHS map out a course to make improvements by creating strategies to make a positive and sustainable change in your communities.  If you want to see the questions in American Sign Language, please watch the video with each question.

Social Services Block Grant - Request For Funds opportunity

The state of Indiana, Deaf and Hard of Hearing Services has had a grant for providing statewide case management services for individuals who need communication accommodations to benefit from these services.  Agencies applying need to be able to provide statewide coverage and have staff who can communicate in various communication modes (American Sign Language, Oral/lipreading, Signed Exact English, Technological apps, etc), DHHS does not pay for interpreters.  DHHS’ current grant will expire on Sept. 30, 2022 and therefore we are seeking bids for the next grant starting on Oct. 1, 2022.   • Deaf consumers and interpreters are encouraged to provide feedback and recommendations on the following form to assist DHHS in improving services to Hoosiers who are deaf or hard of hearing. feedback form
  • At this time the feedback/recommendation form is not able to be completed online. To submit the form, please print the completed from and scan or mail it to DHHS at DHHSHelp@fssa.in.gov or Deaf and Hard of Hearing Services, 402 W. Washington St., MS 23 IGCS, Rm. W453, Indianapolis, IN 46204

State agency interpreter/CART request

Before sending the State agency communication form, please perform the following steps.

  1. Fill out the form
  2. Save the form on your computer
    1. Click file, save as: name the document (name document with consumers last name and date of appointment; e.g., Smith 12.31.2010)
    2. Be sure to note where you are saving the file (such as on your desktop)
  3. Close the form
  4. Go to your email and attach the document
  5. Send the attachment to DHHSHelp@fssa.IN.gov

Please note: Once your file is saved on your computer, you will not need to retrieve the form from the DHHS website, but can open the file you have saved and simply modify the form for a new request (change the name of the document, e.g., Smith 1.15.2011).
