A private employment agency is any person, firm, limited liability company, or corporation, who for hire or with a view to profit offers an individual help to secure employment through:
- Card, circular, pamphlet, or any medium whatsoever
- Display of a sign or bulletin
- Information as to where to find employment
To obtain a new license from the Indiana Department of Revenue (DOR) to operate in the State of Indiana as a private employment agency, the following is required:
- Application for Employment Agency License
- Surety Bond
- Business Tax Registration (INBiz)
- Out-of-State Corporation Notice
- Application for Certificate of Authority of a Foreign Corporation (out of State Corporation)
- Power of Attorney (POA-1)
- Credit Report
- Fee Schedule/Refund Policy
- Personal Financial Statement
- $150 Annual Fee
To renew an existing private employment agency license, the following is required:
- Application for Employment Agency License
- Surety Bond
- $150 Annual Fee
- Fee Schedule/Refund Policy (only if there are changes)
