Steps:

  • Use the subject field to select the correct category for your email.
  • If you have a payment or billing inquiry please send to either the business tax or individual income tax category depending on which type of account you’d have payment or billing questions about.
  • If you are contacting us as a business or organization, please indicate the name of your business or organization in the comments field. 
  • Type your request or question in the comments field and then click submit.
Public records requests can be made via mail or email. 
 
Note: Confidential and personal information such as Social Security numbers, dates of birth or bank account numbers should not be transmitted via DOR's online form, as privacy cannot be protected or ensured. If you must include this information with your inquiry, you may call DOR at (317) 232-2240 and a representative will assist you.
