Address or Narrative Description – Provide an address or narrative description for the location of the project.
City – Provide the name of the city where the project is located.
Nearest Major Intersection – Provide the street names of the nearest major intersection in relation to the project location.
County – Provide the name of the county where the project is located.
Waterbody – Provide the waterbody that will be affected by the project. If you are unsure or the waterbody does not appear to have a name, list “Unnamed” in the space provided.
Latitude and Longitude – Provide a latitude and longitude for the project site. If the site location is large, you may provide an upstream and downstream latitude and longitude. A latitude and longitude can be obtained from multiple websites such as Google Maps or Google Earth. Please provide this information in either decimal degrees or degrees-minutes-seconds format.
Special Site Information - If applicable, provide any details about the site that might be helpful for staff to conduct a site inspection. Examples include a desire to be present during the site inspection, the presence of a dog or other animal at the site, security restrictions or check-in points that need to be addressed prior to entering the site.
