Floyd County Health Department regulates Tattoo, Body Piercing, and Micro-blading Facilities in Floyd County. Please see below for the required forms and documents to acquire a license for your facility and individual licenses. Our licenses are valid from January to December of each calendar year. Our department also issues licenses for Guest Licenses (30-day term) and Temporary Events.
