Floyd County Indiana Birth Certificates

Please Note: We can only provide a copy of a birth certificate for a birth that occurred in Floyd County.  If the person lived here but was born outside of Floyd County, we will not have access to that birth certificate.

You may order a certified copy of a birth, death or marriage record online using your credit card through VitalChek

Obtaining a Birth Certificate by Mail

Thank you for your interest in obtaining a birth certificate in Floyd County Indiana. Please take a moment to review the following information.

Certified birth certificates are issued only to:

  • Person named on the record who is over 18 years of age
  • Parent(s) of person named on the record (A step-parent is not eligible to obtain a birth certificate unless they have Court appointed guardianship papers.)
  • Grandparents(s) of person named on the record
  • Spouse of person named on the record (copy of marriage license may be requested)
  • Adult child of the person named on the record
  • Legal guardian of person named on the record (Legal guardianship document needed.)
  • Sibling, over 18 years of age, of person named on the record.

Complete all items 1 through 13 on our application form.  You may print this form off from the bottom of this page.

Provide all the information you have available to identify the birth record. If the information you furnish is incomplete or inaccurate, we may not be able to locate the record.

If the registrant has been adopted, make the request in the adopted name.

The authorized individual requesting the certified birth certificate must sign the application in front of a Notary Public. Notary Public must complete bottom section of application, must include Seal. (To find a Notary Public, see your local yellow pages or call your banking institution.)

Submit $15.00 for each copy requested. Indicate the number of copies you want and include the correct fee(s) in the form of a cashier's check or money order. PERSONAL CHECKS WILL NOT BE ACCEPTED.

Make cashier's check or money order payable to FLOYD COUNTY HEALTH DEPARTMENT.

Include a self-addressed, stamped envelope with application.

Return Mail Option: Completed certificates are returned using the U.S. Postal Service. If you prefer priority return mail service, you must include a postage paid priority mail envelop.

Make sure that your application has been filled out completely, signed in front of a Notary Public, fee is included and self-addressed, stamped envelope. Incomplete applications will not be processed. 

The Floyd County Health Department now accepts credit cards, with an additional fee.  You may either come to the Health Dept offices and present your credit card, or you may use the VitalChek link on the top right of this screen and conduct the whole process on-line.  The Floyd Co Health Dept still generates the official Birth Certificate for you, i.e. you cannot print it out on-line, but if you want to pay for overnight delivery of the Certificate, we can do that through VitalChek
