Please complete and submit the below form to to request a refund of an administrative penalty that was assessed on a vehicle title, registration, or credential between July 1 and September 1, 2020. Credit card transaction fees applied during checkout in myBMV or paid as part of an automated phone transaction on or after July 1, 2020 are not eligible for a refund.
Refund requests for any other reason will not be considered if submitted through this form. If you would like to request a refund for any other reason, please complete Application for Refund of Certain Fees – State Form 56165.
