INDOT Electronic Permit System Pre-Registration Form

For those customers who are not subscribers with accessIndiana that should first pre-register with INDOT. If for some reason you are denied access to the EPS system then you may not want to spend the $50.00 for an accessIndiana annual subscription. After you submit the prepared form, wait for a response from INDOT prior to subscribing to accessIndiana.
