Individual Beauty Culture Application Instructions
- General Information for Individuals
The Fair Information Practice Act: In compliance with Ind. Code 4-1-6, this agency is notifying you that you must provide the requested information, or your application will not be processed. You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record. Your examination scores and grade transcripts are confidential except in circumstances where their release is required by law, in which case you will be notified.
Mandatory Disclosure of U.S. Social Security Number: Your social security number is being requested by this state agency in accordance with Ind. Code 4-1-8-1 and 25-1-5- 11(a). Disclosure is mandatory, and this record cannot be processed without it. Failure to disclose your U.S. social security number will result in the denial of your application. Application fees are not refundable.
Abandon Applications: If an applicant does not submit all requirements within one (1) year after the date on which the application is filed, the application for licensure is abandoned without any action of the Board. An application submitted after an abandoned application shall be treated as a new application.
Beauty Culture Instructor Applicants: All beauty culture instructor applicants must hold a primary beauty culture license (a cosmetologist, an electrologist, a manicurist, a barber, or an esthetician license) issued by the Indiana State Board of Cosmetology and Barber Examiners in order to meet requirements for the instructor license. All beauty culture instructor licensees must also maintain their primary beauty culture license for the duration of holding their beauty culture instructor license.
Electrologist Applicants: All electrologist applicants must hold a primary beauty culture license (cosmetologist or an esthetician license) issued by the Indiana State Board of Cosmetology and Barber Examiners in order to meet requirements for the electrologist license. All electrologist licensees must also maintain their primary beauty culture license for the duration of holding their electrologist license.
- Preliminary Education Requirements
The below chart defines the preliminary education requirements for all individual beauty culture license types:

Cosmetologist Barber Esthetician Manicurist Electrologist Beauty Culture Instructor One (1) year of licensed practice is equal to one hundred (100) hours of education to an applicant who has completed a minimum of one thousand (1,000) hours of education. One (1) year of licensed practice is equal to one hundred (100) hours of education to an applicant who has completed at least one thousand (1,000) hours of education. One (1) year of licensed practice as an esthetician is equal to one hundred (100) hours of education to an applicant who has completed at least four hundred (400) hours of education. (1) For an applicant with less than twenty (20) years of actual licensed practice as a manicurist, one (1) year of licensed practice is equal to one hundred (100) hours of education to an applicant who has completed at least three hundred (300) hours of education.
(2) For an applicant with twenty (20) or more years of actual licensed practice as a manicurist, one (1) year of licensed practice is equal to one hundred (100) hours of education to an applicant who has completed at least one hundred (100) hours of educationOne (1) year of licensed practice as an electrologist is equal to one hundred (100) hours of education to an applicant who has completed at least two hundred (200) hours of education. One (1) year of licensed practice as a beauty culture instructor is equal to one hundred (100) hours of education to an applicant who has completed at least seven hundred (700) hours of education. - Examination Applicants
Submit the following items with your online application if you meet the preliminary and beauty culture education requirements:
- Application Fee of $40.00: Pay by credit or debit card. All application fees are nonrefundable.
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement.
- Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
- Education or Apprenticeship:
- Beauty Culture Education Certification: Your Indiana approved beauty culture school must complete the Beauty Culture Education Certification Form and return to you and you will upload the document as part of the application process; or
- Department of Labor Registered Apprenticeship Program: If you completed a Department of Labor registered apprenticeship program, please provide your apprentice number and upload a copy of your certificate of completion.
- Examination Pass Notice: You must pass the written examination through PSI [those that have graduated before the exam was part of graduation (7/1/2013) will need to apply for a Waiver of the 3 year deadline if they failed to meet it.] and upload your examination pass notice or submit to our office as part of your application.
- Reciprocity Applicants (Licensed in Another Jurisdiction)
To apply by reciprocity, a person must possess a valid license from another jurisdiction to perform acts that require a license and the jurisdiction issuing the license must impose substantially equal requirements on applicants for the license as are imposed on applicants for an Indiana license. (The number of hours reflects the applicant’s state’s minimum education requirement rather than the actual number of hours that were obtained. If the applicant completed extra hours above and beyond the state’s minimum requirement, the extra hours would not apply.) If the jurisdiction issuing a license does not impose substantially equal requirements for education hours, the board may approve the combination of education hours plus actual licensed practice in the other jurisdiction. The jurisdiction must also require at least 70% passing on the written and practical exams. Please review the Beauty Culture Education Requirements.
If the applicant’s jurisdiction does not issue a license to practice, the applicant may apply for a provisional license (see below).
Submit the following items with your online application if you meet the minimum reciprocity requirements:
- Application Fee of $40.00: Pay by credit or debit card. All application fees are nonrefundable.
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement.
- Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
- Verification of State Licensure(s): Request a license certification from the state(s) or country you hold, or have held, a license in. (if from a foreign country, must include an English translation of the documents). Online certification is acceptable if it provides a field for possible disciplinary action. All forms of certification must come directly from the state/country and may be sent via email to pla12@pla.in.gov. A copy of your license will not suffice.
- Provisional Applicants (Not Licensed in Another Jurisdiction)
If an applicant comes from a jurisdiction that does not issue a beauty culture license, the board may issue an initial provisional license to an applicant who meets sufficient training or experience and if a license an applicant is granted an initial provisional license, must work under the supervision of a licensed beauty culture professional as defined below. (Provisional Licenses are not granted for Beauty Culture Instructors)

- Application Fee: $40.00 - Pay by credit or debit card. All application fees are nonrefundable.
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement.
- Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
- Proof of Training/Experience: Applicant must provide proof of experience, such as proof of salon experience. Applicant must submit transcript of training from a beauty culture school. All transcripts must be accompanied by a certified translation in English.

- Beauty Culture Provisional Verification: Section A - Experience in Your Jurisdiction and the Indiana Supervising Licensing and Beauty Culture Salon Sections are required for Provisional License Applicants.
- Upgrade from Provisional Applicants
Submit the following with your online application if you have met the minimum supervised experience by an Indiana licensee under your Provisional License:
- Application Fee: $40.00 - Pay by credit or debit card. All application fees are nonrefundable.
- Positive Response Documentation: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement.
- Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree.
- Beauty Culture Provisional Verification: Section B - Provisional Upgrade Supervisor Verification of Experience is required for Upgrade Applicants.
Beauty Culture Facility Application Instructions
- General Information for Facilities

You have the right to challenge, correct, or explain information maintained by this agency. The information you provide will become public record.

- Beauty Culture Salons Submit the following with your online application and review the requirements:
- Application Fee: $40.00 - Pay by credit or debit card. All application fees are nonrefundable.
- Beauty Culture Salon Minimum Requirements - The Inspection Checklist Example is available for review.
- Applicant must select a site for the salon; if the site is in the same building as a residence, it must be separated from the residence by a substantial floor to ceiling partition and have a separate entry.
- Applicant must obtain any building permit, certificate of occupancy, or other approval action required under IC 22-15-3 and IC 36-7-4 to operate the cosmetology salon.
- Applicant must install the furnishings and obtain the salon equipment required under rules adopted by the board.
- Please DO NOT apply for the salon license until your facility is ready to open.
- Temporary permits are issued upon receipt of a complete application and are valid for three (3) months. If your salon is not ready to open when you apply, you are limiting the time your salon may operate on the temporary permit and you may have to close until your salon license is issued. Operating a salon without a temporary permit will result in your application for license being denied.
- Please DO NOT allow individuals that are not licensed as Indiana beauty culture professionals to practice beauty culture in your salon. Doing so will result in denial of your application for salon license.
- All beauty culture professional licensees must post their Indiana license at their workstation. Expired licenses posted at the salon will result in your application for salon license being denied. You may check the status of a license on our website using the Free Search and Verify page.
The beauty culture salon must be under the personal supervision of a person who is licensed as a beauty culture professional. - Mobile Salons Submit the following with your online application and review the requirements:
- Application Fee: $40.00 - Pay by credit or debit card. All application fees are nonrefundable.
- Mobile Salon Minimum Requirements:
- A mobile salon/shop may be;
- (1) a self-contained facility that may be moved, towed, or transported from one location to another and in which cosmetology, electrology, esthetics, manicuring or barbering is practiced, or;
- (2) A business in which cosmetology, electrology, esthetics, manicuring, or barbering equipment is transported to and used on a temporary basis at a location other than a selected salon site, including:
- (A) other cosmetology, electrology, esthetic, manicuring, or barbering salons/shops;
- (B) clients' homes; and
- (C) nursing homes.
- (2) A business in which cosmetology, electrology, esthetics, manicuring, or barbering equipment is transported to and used on a temporary basis at a location other than a selected salon site, including:
- (1) a self-contained facility that may be moved, towed, or transported from one location to another and in which cosmetology, electrology, esthetics, manicuring or barbering is practiced, or;
- Applicants must comply with the rules adopted by the board (820 IAC 2-1, 3-1, and 3-2)
- A mobile salon/shop may be;
The mobile salon must be under the personal supervision of a person who is licensed as a beauty culture professional. - Tanning Facilities
Submit the following with your online application:
- Application Fee: $200.00 - Pay by credit or debit card. All application fees are nonrefundable.
IC 16-1-47, Section 18(A) the operator of a tanning facility must report each known: (1) injury; or (2) complaint of an injury; that results from the use of a tanning device in the tanning facility to the State Board.
- Beauty Culture Schools
Submit the following with your online application:
- Application Fee: $400.00 - Pay by credit or debit card. All application fees are nonrefundable.
- Additional Documentation to Upload:
- The contract which is to be used between the beauty culture school and its students which must include the school's refund policy;
- A copy of the beauty culture school's deed for the property where the cosmetology school is located or a lease for at least one year showing that the premises are to be used as a cosmetology school;
- A budget for the first year of operation of the cosmetology school;
- A financial statement on a form provided by the board: Financial Statement Supplemental Form
- A floor plan of the beauty culture school of which shall show, in detail, the following;
- The placement of students
- Classroom entrances and exits
- Space for waiting customers (indicating number of chairs)
- Separate student and public cloakroom spaces
- Two separate restrooms
- A list of licensed instructors to be added on the online application; and
- Fire Marshal Report
Monthly Verified Reports
Pursuant to 820 IAC 4-1-10, beauty culture schools, no later than the 15th day of each month, shall submit a verified report to the board including:
- The names of all new students with
- Tuition Owed
- Total Hours Accrued
- Status of Enrollment
- Course Enrolled
- Last Date of Attendance
The form can be downloaded here, and must be uploaded via your MyLicense One account. Please ensure you have linked your Beauty Culture School license with the license number and registration code (please contact pla12@pla.in.gov if you need your Registration Code) and then you will use the Upload Additional Documentation Action Item to upload the completed form.

Renewal Instructions
- License Renewal Information
Renewal notices are sent approximately ninety (90) days prior to the expiration date. Licensees with a valid email address on file will be emailed the renewal information. Licensees that do not have a valid email address on file will be mailed license renewal information. This notice is mailed to the licensee's address of record with the Board. - Reinstating a Beauty Culture Professional License
IC 25-1-8-6 provides for the reinstatement of a beauty culture professional license that has been expired beyond three (3) years. If you would like to reinstate your beauty culture professional license that has been expired for more than three (3) years, please follow the steps below:
- Payment of the reinstatement fee of $80.00 (Cost of the current renewal fee of $40.00 plus cost of the current application fee of $40.00). All fees are non-refundable.
- Complete such remediation and additional training as deemed appropriate by the board given the lapse of time involved
- Expired five (5) or more years:
- The board has determined that taking and passing the written licensing examination is appropriate remediation for those expired for five (5) or more years).
- Schedule your examination using the registration information contained in the eligibility letter that you will receive upon receipt of the application.
- Take and pass your examination within one year of application to reinstate your license. Log back into your MyLicense One account and use the Upload Additional Documention action item.
- The board has determined that taking and passing the written licensing examination is appropriate remediation for those expired for five (5) or more years).
If your license cannot be verified by the State Board of Cosmetology and Barber Examiners and you do not have an actual expired license to submit, you will need to complete the appropriate course at an approved Indiana beauty culture school and apply for licensure by examination.
- Reinstating a Beauty Culture Facility License

Once your reinstatement has been submitted online, you will have an inspection scheduled for your facility in order to complete the reinstatement process.
