Who We Are
The St. Joseph Department of Health is committed to supporting the well-being of county residents through our Community Health Worker (CHW) program. Our certified CHWs are licensed insurance navigators who serve as invaluable connection points, working to establish bridges between community members and available resources in the areas of Maternal Infant Health, Childhood Lead Poisoning Prevention, and Chronic Disease Prevention and Education. Through their dedication and commitment, these health workers play a crucial role in creating a healthier and more connected community within St. Joseph County.
The survey for resource navigation assistance is now disabled as we prepare to launch our new Chronic Disease CHW program in the first quarter of 2026, which will be dedicated to providing free community health screenings, connection to primary care services, and health education for chronic disease prevention.
