 Possible Points
The following criteria will be used to review all submitted applications: 
Completeness of application: All sections of application completed with thorough information and clear description of project, needs, and outcomes.  15
Application clearly describes the organization and demonstrates expertise in the Core Public Health Service area proposed.15
Application clearly demonstrates how the intended project/activity and outcomes may improve the delivery or outcomes of the Core Public Health Services and, ultimately, better health measures.15
Application clearly demonstrates a history in providing similar services or related work. 22.5
  
Project proposal:  
Application demonstrates a clear purpose, scope, deliverables and timeline. 22.5
Application demonstrates a clear connection between proposal and meeting the delineated key performance indicators.22.5
Application demonstrates appropriate project scaling to population served.15
Application demonstrates inclusion and description of partners, if applicable (or an explanation why a stand-alone project could achieve similar outcomes).15
Application demonstrates use of evidence-based programming. 22.5
Application demonstrates measurable, specific improvement objectives in Core Public Health Service(s) on specified population. Outcomes are clearly defined and measurable. 22.5
Proposal has an effective health equity statement that addresses service equity for underserved and underinsured populations. 15
Proposal has a complete sustainability statement, clearly outlining a sustainability plan once the grant period has ended. 15
Application demonstrates a thoughtful and realistic budget. Clear justifications for budgeted activities/items are provided. 22.5
