The Miami County Registrar's Office performs 3 primary services for the public:
- Issuing birth certificates
- Issuing death certificates
- Performing genealogy lookups
Accessing Service
If you want to utilize these services through the mail, you will need to fill out the appropriate form, which can be found here. These services can not be performed online. Emailed requests for these services will not be fulfilled.
Rates
Effective August 1, 2013, the following rates apply to our services:
- Affidavit requesting amendment - $10 per document
- Birth certificates - $10 per copy for walk-in or mail-in requests
- Certified Copy of Paternity Affidavit - $5 per copy
- Death certificates - $15 per copy for walk-in or mail-in requests
- Genealogy Search - $5 per search
- Genealogy Uncertified Copy of Document - $10 per copy
- Paternity Affidavit upon marriage - $20 per document
- Paternity Affidavit - $30 per document
All fees are subject to change without notice.
Warning: False application, altering, mutilating, or counterfeiting Indiana birth certificates is a criminal offense under I.C. 16-37-1-12.
