Childhood Immunizations
Appointments for childhood immunizations can be made at (765) 472-3901 x1216.
Childhood Immunization Schedule
We are able to immunize children with Private Insurance and/or following VFC guidelines:
Children under 19 years of age are eligible to receive vaccines through the VFC program if they meet one of the following criteria:
- Eligible for Medicaid
- Uninsured
- Under insured
- American Indian or Alaskan Native
Underinsured means the child has health insurance, but the insurance either doesn’t cover the recommended vaccine(s) or only covers a fixed dollar amount for vaccines. A child becomes eligible once the fixed dollar amount is reached.
